Student Complaints and Grievance Reports

The Dean of Students Office is here to help. There are multiple ways to report a concern. Any UK community member or concerned citizen external to the institution can submit a concern through one or more of the reporting mechanisms found below. We also understand that situations and circumstances maybe complex. That said, please know that all offices identified as a safety resource for the UK community work very closely together. Should there be aspects of your report that we believe is best addressed in a different office, we will make sure to share the report with the appropriate office. Community of Concern Report

This form is for reports of individuals who may present as a threat to the UK community.

Center for Support and intervention and TRACS Report

This form is for reporting students or groups of students who appear to be experiencing social, national or mental wellness crises.

Campus Security Authority Form

This form is a campus crime statistics form and is not designed to take the place of an official police report.

Student Conduct Report

This report is for students or organizations who you believe may have violated the student code of conduct.

University Concern

The University of Kentucky maintains a decentralized complaint process.  Each unit or department maintains records of complaints.  A centralized webpage for complaints is maintained on the Dean of Students website to help students locate the proper unit for the complaint.  If the student cannot determine the proper unit to register the complaint, a general complaint form is available.

Institutional Equity and Equal Opportunity and Bias Incident Report

This report is for any actions or behaviors that are perceived as discrimination or harassment under Title IX.

University of Kentucky Police Department

This report is for immediate threats of harm or danger, criminal behavior and suspicious behavior.

Hazing Report

This report is for any concerns of alleged hazing in student organizations.

Office of Legal Services

This resource is for reporting and ethical issues, serving subpoena’s or filing an open records request.
