1. Access the online Student Affiliation Form using your MyUK LinkBlue username and password.
3. Select Start New Application and complete the form to the best of your knowledge. The items marked with a red asterisk are required fields.
4. Select Save Application Draft to proceed to the next section of the form. Upload documentation of a disability or condition to the online form. Please note, documentation is required prior to the provision of permanent accommodations. If you have questions regarding documentation, please email drc@uky.edu.
5. When you are done, select Submit Application. You will receive an email notification confirming your submission through your official UKY email.
6. Within 1-3 business days, you'll receive an email with further instructions from our office to schedule a meeting with a Disability Resource Center Consultant to finalize accommodations.
