How to Complete the Online Affiliation Form
- Access the online Student Affiliation Form using your MyUK LinkBlue username and password.
- Select Start New Application and complete the form to the best of your knowledge. The items marked with a red asterisk are required fields.
- Select Save Application Draft to proceed to the next section of the form. Upload documentation of a disability or condition to the online form. Please note, documentation is required prior to the provision of permanent accommodations. If you have questions regarding documentation, please email drc@uky.edu.
- When you are done, select Submit Application. You will receive an email notification confirming your submission through your official UKY email.
- Within 1-3 business days, you'll receive an email with further instructions from our office to schedule a meeting with a Disability Resource Center Consultant to finalize accommodations.
